Septal Extension Graft in Korea: Who It Suits, Risks
Have you read that Korean clinics use a septal extension graft for almost every tip, and wondered whether you actually need one? Many people who message us have a short or upturned nose, or a tip that sits flat, and they want to know what this graft does before they agree to it.
By the end of this guide, your questions about rhinoplasty should turn into a clear next step.

Send my photos on WhatsAppThis guide explains what the graft is, why Korean rhinoplasty uses it so often, which cartilage it is made from, who it suits and who it doesn't, the risks, and what recovery looks like.
Hello, I'm Dr. Choi Hang-seok, Head Surgeon at TIANA Plastic Surgery in Gangnam, Seoul.
TIANA performs only rhinoplasty and revision rhinoplasty. I handle each patient one-to-one from consultation through surgery, anesthesia and aftercare, and every nose is mapped with 3D-CT imaging before surgery.
The short answer: a septal extension graft is cartilage fixed to the end of your septum so the tip has a firm base. It lets the surgeon set tip projection, nose length and tip angle, which is why it is common in short nose rhinoplasty. It is not needed for every nose. A tip that only needs refining, or a nose with very little cartilage and tight skin, may call for a different plan.
What a septal extension graft is

The septum is the wall of cartilage and bone that divides the inside of your nose. Its front edge ends just behind the tip. In many noses that edge is short or soft, so the tip has little to rest on.
The graft is a strip of cartilage stitched to that front edge to make it longer and stronger. The tip cartilages are then fixed to the graft. Because the tip now sits on a firm post, its position can be set more precisely than with stitching alone.
Three things can be changed this way:
- Tip projection: how far the tip stands out from the face.
- Length: how long the nose looks from the side, from between the eyes to the tip.
- Tip angle: whether the tip points up, straight ahead or slightly down, which also changes the angle between the nose and the upper lip.
Why Korean rhinoplasty uses the septal extension graft so often
Many East Asian noses have a short septum, small tip cartilages and thicker skin over the tip. Thick skin needs a strong frame underneath, or the tip stays flat and round however carefully it is stitched.
That is the main reason a septal extension graft appears in so many Korean case plans. It gives the tip support that the nose's own cartilage cannot provide. It is also central to short nose rhinoplasty, because lowering an upturned tip means building forward and downward from the septum.
Doctor's note
In consultation, patients often ask for the tip "a little higher" while showing a photo of a longer, more defined nose. Projection, length and angle move together on the graft. I set them as one plan with the bridge, so the nose does not end up tall but short, or long but flat.

Example case (bulbous tip, short nose): side profile before surgery (left) and 3 months after (right). Procedures: silicone bridge implant, tip with septal and donor rib cartilage, bulbous tip refinement, short-nose correction, osteotomy, columella and nasolabial angle adjustment. Results vary between individuals; swelling, bruising, asymmetry and infection are possible risks. Shared with the patient's consent.
How the tip is planned on its own, and when graft work is not needed, is on the tip plasty in Korea page.

Ask which procedure fits meWhich cartilage is used
The graft has to be straight and strong, so the source matters. The table shows what TIANA uses and when.
| Cartilage | When it is used | What to know |
|---|---|---|
| Septal cartilage | First choice when there is enough | Straight and strong; taken from inside the nose, so the amount is limited |
| Ear cartilage | Shaping and small additions at the tip | Curved and softer; rarely strong enough to be the main support alone |
| Your own rib cartilage | Small or weak septum, revisions, large changes | Plenty of material; leaves a small chest scar and adds some recovery |
| Donated rib cartilage | When more cartilage is needed without a chest incision | No chest scar; how it behaves over time is discussed in consultation |
Which one suits you depends on how much septum you have, whether it has been used before and how much the tip needs to move. The 3D-CT and examination answer most of that before surgery. More on rib cartilage is on the rib cartilage rhinoplasty in Korea page.
I talk through common worries about donated rib cartilage in this video (the video is in Korean).
Who a septal extension graft suits, and who it doesn't
A graft is a tool for moving the tip. If your tip does not need to move much, it may add stiffness and recovery for little gain.
| Your situation | Septal extension | Often a better fit |
|---|---|---|
| Short or upturned nose, wide nasolabial angle | Often central to the plan | Columella work may be added |
| Flat tip with thick skin | Often helps projection hold | Tip refinement on top |
| Tip only slightly round, good length | Usually not needed | Tip plasty without a graft |
| Tip turned up after earlier surgery (contracture) | Often used, with care | Revision plan with rib cartilage |
| Very little cartilage and tight, scarred skin | Movement is limited | Smaller goal, or tissue settling first |
| You want a soft, very mobile tip | Expect a firmer tip | Discuss priorities first |
If your main concern is a tip that points up, how an upturned nose is corrected explains the causes. If the area under the tip looks hidden or heavy from the side, read columella surgery in Korea and why lower isn't always better, because the graft and the columella are planned together.
Septal extension graft risks
The American Society of Plastic Surgeons lists swelling, bruising, asymmetry, infection, breathing difficulty and the possible need for revision among rhinoplasty risks. With this graft, a few more are worth knowing by name.
- A stiff nose tip. The tip rests on a firm post, so it moves less than before. This is expected. It is a trade-off to accept or discuss, not a sign that something went wrong.
- Deviation. If the graft is fixed off-center or the septum leans, the tip can tilt to one side.
- Rib cartilage warping. Rib cartilage can bend slightly after it is carved, which can tilt the tip over time. Carving and fixing are planned to reduce this, but it cannot be ruled out.
- Nasal blockage. A graft that is thick, or sits on a septum that already leans, can narrow the airway on one side.
- Over-lengthening. A tip pushed too far down looks long and heavy. A modest change that suits the face usually looks better.
- Infection or exposure. Uncommon, but a graft under tight skin can cause redness or, rarely, show through.
These happen in a minority of cases. What lowers the chance is mostly planning: a straight graft fixed on the midline, a change sized to what the skin allows, and checking the airway before surgery.

Example case (revision rhinoplasty): side view before surgery (left) and 2 weeks after (right). Procedures: revision rhinoplasty, silicone bridge implant, tip with septal and donor rib cartilage, bulbous tip refinement, short-nose correction, osteotomy. Results vary between individuals; swelling, bruising, asymmetry and infection are possible risks. Shared with the patient's consent.
Recovery, stitches and revision
Recovery after this surgery follows the same timeline as most tip work at TIANA. The tip often feels firm and numb at first, and it softens a little as swelling settles over the following months.
| Milestone | Usual timing |
|---|---|
| Splint off and outer stitches out | Around day 7 |
| Inner stitches out | Around day 14 |
| Major swelling settles | 1 to 2 weeks |
| Looks natural to others | Around 3 to 4 weeks |
Plan about 7 to 14 nights in Seoul, and at least 7. High-concentration oxygen care is used after surgery to help swelling settle, and recovery differs between people. If you fly home around day 7, you leave with a referral letter and a surgery certificate so a local doctor can remove the inner stitches.
When revision is needed
Revision rhinoplasty after a septal extension graft is usually about one of four things: a tilted tip, a tip that is too long, a tip that is too stiff, or a nose that turned up again as scar tissue tightened. TIANA also treats noses after surgery elsewhere.
How much can be changed depends on the cartilage left and the state of the skin. Rib cartilage is often needed in a revision because the septum has already been used. If you are planning a revision trip, revision rhinoplasty surgeons in Korea and how many days to plan covers the timing.
More articles are collected in all short and upturned nose articles, and the overall design of bridge, tip and nostrils is on TIANA's English rhinoplasty page.
Summary
- A septal extension graft is cartilage fixed to the end of the septum so the tip has a firm base.
- It sets tip projection, length and tip angle, which is why Korean short nose rhinoplasty uses it often.
- Septal cartilage comes first; ear cartilage helps with shaping; rib cartilage, your own or donated, is used when more is needed.
- It is not for every nose. A tip that needs only refining may not need one.
- Risks include a stiff tip, deviation, rib cartilage warping, nasal blockage and over-lengthening. Careful sizing and fixing lower them.

Frequently asked questions
Q1What is a septal extension graft?
It is a piece of cartilage fixed to the end of your own septum so it reaches further toward the tip. The tip cartilages are then attached to it, which lets the surgeon set how far the tip projects, how long the nose looks and which way the tip points.
Q2Does a septal extension graft make the tip stiff?
Usually the tip feels firmer than before, because it now rests on a solid support. Most people get used to it, and some softening happens as swelling settles. If moving the tip freely matters a lot to you, say so in consultation, because it can change the plan.
Q3Which cartilage is used for a septal extension graft?
Septal cartilage is the first choice when there is enough of it. Ear cartilage can help with shaping, and rib cartilage, either your own or donated, is used when the septum is small, weak or already used in an earlier surgery.
Q4Can a septal extension graft be revised?
Yes. A graft that has tilted, a tip that is too long or too stiff, or a nose that turned up again can be corrected in revision rhinoplasty. The plan depends on how much cartilage is left and how the skin has healed, so it is decided after examination and 3D-CT.
Q5How many nights should I stay in Seoul after septal extension?
Plan about 7 to 14 nights, and at least 7. The splint and outer stitches usually come off around day 7 and the inner stitches around day 14. If you fly home around day 7, you leave with a referral letter and a surgery certificate.
