By the end you'll know what the consultation is, how booking runs, who it suits, what decides the cost and which review claims to trust, so you can choose a next step that fits the nose and the family budget.

Hi, this is Dr. Choi Hang-seok, Head Surgeon at TIANA Plastic Surgery in Seoul.

TIANA works only on noses. Booking runs through WhatsApp with replies in English, and I'm your surgeon from consultation through aftercare. I manage the anesthesia as well.

I wrote this because many people assume that asking for an appointment commits them to surgery, while others book flights first and only then learn that the tip was never the whole job.

Booking has two stages. A photo consultation on WhatsApp comes before you travel, and an in-person consultation with 3D-CT comes on day 1 in Seoul, with design and cost confirmed before surgery.

To begin, send a front-view photo of the nose on WhatsApp and ask whether this looks like a tip concern alone or the bridge as well.

What is a bulbous tip consultation, and how does it work?

In short, it's the step that sets the scope of surgery for a round, heavy-looking tip. Each stage answers a different question.

The photo stage asks what is likely. You send 5 photos (front, 45-degree and side) on WhatsApp, and the reply in English explains the likely scope of surgery and how the estimate is worked out. We don't offer video-call consultations, so photos are the way to start from home.

The in-person stage asks what is actually there. Photos show the outline of a tip, while the cartilage, the bone and the nasal structure need an examination, which is where 3D-CT imaging comes in. If you bring a photo of a nose you like, I compare it with your anatomy and tell you which parts can be reached naturally.

Who this consultation suits, and the methods it can lead to

Put simply, it suits anyone whose main concern is the tip, including people who aren't sure yet that the tip is the real issue. Here is how common concerns usually map to methods:

Rhinoplasty example case, bulbous tip, front view before and after

Example case (bulbous tip): front view before surgery (left) and 2 weeks after (right). Procedures: silicone bridge implant, tip with septal and donor rib cartilage, hump reduction, bulbous tip refinement, drooping-tip correction, columella and nasolabial angle adjustment. Results vary between individuals; swelling, bruising, asymmetry and infection are possible risks. Shared with the patient's consent; eyes and mouth are pixelated for privacy.

  • A round, heavy tip: bulbous tip refinement, which brings the tip cartilage together. This explanation of how surgery narrows a round, wide tip covers the anatomy.
  • A tip needing only light refinement: minimal-incision tip work or tip surgery without an implant, which may be enough. These tend to recover faster and generally change less.
  • A long or drooping tip: adjusting tip height and angle. Septal extension is the method for a short or upturned tip, and it uses cartilage to bring the tip down.
  • A low bridge as well: full rhinoplasty, with a bridge implant plus tip support designed as one profile.
  • Thick skin: the same methods apply, with a limit on how defined the tip can look. It's worth reading what tip surgery can change under thick skin first.
  • A previous nose surgery: revision rhinoplasty, including difficult cases such as contracture.

Say you're arranging this for your mother, who is 58, with a round tip that has drooped over the years and a bridge she has always liked. I'd want to know two things before anything else. Is the droop the main bother, or the width? And what's on her medication list, since blood pressure treatment and blood thinners change the planning far more than her age does.

In that situation, I'd also suggest you ask what language help is available on the day, and plan to be in the room if she'd feel better with you there. Then her five photos and her list of medications are what you'd have ready.

Doctor's note

The nose you're consulting about belongs on a face that has lived for decades, and raising or narrowing only one part can make it stand out. I value a nose that blends in over a dramatic change, more so as people get older. Coming to the consultation with one clear sentence about what bothers you helps more than a folder of celebrity photos.

Wondering which type your nose is?

Share your profile and front photos — the surgeon will tell you which procedures your nose would actually need. No cost, no obligation.

How does booking work from first message to surgery day?

The order is short, and you can stop at any point before you book surgery.

  1. Message on WhatsApp. Send the 5 photos and your question.
  2. Read the reply. It covers the likely scope and how the estimate is worked out. A deposit is only discussed once you decide to book surgery, so it's fine to just ask questions first.
  3. Choose your dates. Plan about 7 to 14 nights in Seoul, at least 7. We're open 10:00 to 19:00 on weekdays and 10:00 to 16:00 on Saturdays, and closed on Sundays and public holidays, so day 1 needs to fall on an open day.
  4. Before you pay anything, ask for the deposit amount and cancellation terms.
  5. Day 1 in Seoul. You have the in-person consultation and any tests, then design and cost are confirmed before surgery.

The days after surgery are covered in these bulbous tip procedure steps in English, and our English consultation page answers the short questions on stay and downtime.

With that order in mind, add the dates you could travel to your WhatsApp message and ask whether a 7-night or a longer stay would suit the likely scope.

What should you bring to the consultation?

A few items make a Korea bulbous tip consultation booking appointment far more useful:

  • A photo of a nose you like, if you have one
  • The operation details from any previous nose surgery
  • A current medication and supplement list, with any allergies
  • Your return flight date
  • Your written questions, especially who to contact once you're home

A parent who is nervous may forget all of them in the room, so keep the list on your own phone.

What decides the cost you're quoted?

TIANA doesn't publish a fixed menu price, because two similar-looking noses can need very different surgery. The figure moves with these things:

  • The scope of surgery, whether that's the tip alone or the bridge and tip together
  • How much cartilage is available and how thick the skin is
  • Whether it's a revision
  • The length of your stay, which affects your own travel budget

Your final fee is confirmed after the examination in Seoul, so treat the first estimate as a way of understanding the plan. A plan chosen only because it seems low can cost more in money and trips if it needs correcting later. Ask on WhatsApp what it would cost for your case, with the photos attached.

What do reviews get right about tip consultations?

The common claim that a plan changed between the photos and the visit is often true, and it's usually reasonable, because photos can't show cartilage or internal structure. Ask what changed and why.

Another frequent claim is that a good consultation means being told yes. That one doesn't hold up. A surgeon explaining the limits of a thick-skinned tip is giving you useful information. If you're reading whatclinic reviews before a consult, look for what the writer learned about their own nose, since a star rating tells you very little about yours.

The claim that you should fly in for several consultations is partly true. Comparing how surgeons explain their reasoning has value, but it takes days from a trip that's built around healing time.

Who should wait, and what are the risks?

Age alone is rarely what holds someone back. General health and the timing of the trip matter more.

Risks and side effects to know

Swelling, bruising, nasal congestion and a numb or firm tip are common after tip surgery, and they generally fade over weeks to months. Results vary between individuals. The American Society of Plastic Surgeons lists further risks such as infection, bleeding, anesthesia risks, breathing difficulty, an unsatisfactory appearance and the possibility of revision.

As a general rule, postpone and get medical advice if there's an active infection or inflammation on the nose, unstable heart disease or blood pressure, a bleeding problem, or a pregnancy. Mention blood thinners and diabetes in your first message rather than on the day. The examination settles whether surgery suits you or your parent.

A reaction after the flight home calls for a local doctor without delay, particularly with fever, spreading redness, heavy bleeding, chest pain or breathlessness. Anyone flying home around day 7 gets a referral letter and surgery certificate, which should travel in hand luggage. The CDC's medical tourism advice is worth reading together, including its warning that flying after surgery raises the risk of blood clots, which matters more for older travelers.

If you want to see how real noses changed and how many months after surgery each photo was taken, browse TIANA's rhinoplasty case gallery before your consultation.

Your first message can be short

Five photos on WhatsApp and one question are enough: does this nose need the tip alone, or more? You can ask before booking anything, and you don't need dates yet. When you or your parent come to Seoul, I examine the nose with 3D-CT and recommend the treatment that fits, and you decide from there.

Frequently Asked Questions

Q1Do I have to pay a deposit to get a consultation?

No deposit is needed to ask questions on WhatsApp. It is only discussed once you decide to book surgery, and you can ask for the amount and cancellation terms before you commit.

Q2Which photos do you need for a bulbous tip question?

Five photos: front, 45-degree and side views. Daylight and no filters make the tip easier to see, and a single front view is a fine way to open the conversation.

Q3Can my son or daughter message on my behalf?

Yes, a family member can send the photos and questions on WhatsApp in English. Include the patient's medication list and any previous nose surgery so the details are in one place.

Q4Is there a video consultation before I fly?

No. We don't offer online or video-call consultations. The pre-travel step is the photo consultation on WhatsApp, and the examination takes place in Seoul.

Q5Can the consultation and surgery happen on the same trip?

Yes, the usual schedule puts the in-person consultation on day 1, with design and cost confirmed before surgery. Plan about 7 to 14 nights so the follow-up visits fit.
